Policies and consents
How InSitu Clinic handles your information, the terms that govern our service, and what you consent to when receiving care that includes virtual communication.
Privacy Notice
This Privacy Notice explains how InSitu Clinic collects, uses, stores, shares and protects personal information when you use our website, submit an assessment, communicate with us or receive our services. It is intended to meet the transparency requirements of the Nigeria Data Protection Act 2023 and the Nigeria Data Protection Act General Application and Implementation Directive 2025, together with applicable health confidentiality obligations.
1. Who we are
InSitu Clinic provides doctor-led metabolic and weight-management services through digital screening, telehealth, home assessments, laboratory coordination, prescriptions, medication supply coordination, injection support and clinical follow-up.

This notice applies to prospective patients, patients, former patients, people who contact us, website visitors, authorised patient representatives, emergency contacts and other persons whose information is provided in connection with our services.
The standard InSitu Clinic metabolic programme is intended for adults aged 18 years and above. If we process information about a child or a person who lacks legal capacity, we will apply additional safeguards and obtain appropriate authority or consent where required.
3. Information we may collect
We collect only information that is relevant to the purpose for which it is required. Depending on your interaction with us, this may include:
- Identity and demographic information, including your name, date of birth, age, sex, identification details and patient number.

- Payment and transaction information, including invoices, receipts, payment status and limited payment references. Card or bank details may be processed directly by a payment provider rather than stored by InSitu Clinic.
- Communications, including emails, telephone records, WhatsApp messages, appointment requests, complaints and feedback.
- Home-visit and service information, including the identity of persons present, consent records, specimen collection details, delivery information and visit notes.
- Website and device information, including IP address, browser, device type, pages visited, cookies and similar technical data, where enabled.
- Information from authorised third parties, such as laboratories, imaging providers, treating clinicians, pharmacies, hospitals, insurers or a person you authorise to communicate with us.
4. How we collect information
- Directly from you through our website, Tally or other intake forms, telephone, email, WhatsApp, telehealth consultations, home visits and clinical assessments.
- From clinicians, nurses and other professionals involved in your care.

- From payment processors and financial institutions for payment confirmation and reconciliation.
- Automatically through website technologies, cookies and server logs, where applicable.
- From an authorised representative, emergency contact, referring clinician or other person where you have authorised the disclosure or another lawful basis applies.
5. Why we use your information
- To receive and review your preliminary assessment and determine the appropriate next step.
- To verify identity, obtain consent, assess clinical suitability and provide safe medical and nursing care.
- To arrange home assessments, specimen collection, laboratory testing, imaging, consultations, prescriptions, medication supply, delivery and follow-up.
- To communicate appointment information, results, treatment instructions, safety advice, invoices, receipts and service updates.
- To maintain accurate health records and support continuity, quality and safety of care.
- To monitor treatment response, adverse effects, adherence and clinical outcomes.
- To manage payments, accounting, audits, complaints, legal claims, regulatory obligations and professional standards.
- To secure our systems, prevent fraud, investigate incidents and manage data breaches.
- To improve our clinical processes and services through audit, quality improvement and de-identified or aggregated analysis.
- To send optional marketing or educational communications only where you have chosen to receive them or where otherwise lawfully permitted. You can opt out at any time.
6. Lawful bases for processing
Depending on the purpose, we may rely on one or more lawful bases, including:
- Your consent, including explicit consent where required for a specific use of sensitive personal information.
- Taking steps at your request before entering into a service arrangement and performing our service agreement with you.
- Compliance with legal, regulatory, tax, accounting, professional and patient-safety obligations.
- Protection of your vital interests or those of another person in an emergency where consent cannot reasonably be obtained.
- Our legitimate interests in operating a safe, secure and effective healthcare service, provided those interests do not override your rights and reasonable expectations.
- Medical care provided by, or under the responsibility of, professionals and service providers who owe duties of confidentiality.
Withdrawing consent does not affect processing already carried out lawfully. It may also be impossible to delete or stop processing information that we must retain for clinical, legal, professional or safety reasons.
7. Clinical decision-making
InSitu Clinic may use forms, formulas and screening rules to calculate values such as age or BMI and to flag information for review. We do not intend to make treatment or prescribing decisions solely through automated processing. A qualified clinician reviews the relevant information and applies professional judgement.
8. Who may receive your information
We share information only where it is necessary, proportionate and lawful. Recipients may include:
- InSitu Clinic physicians, nurses, clinical coordinators, authorised administrators and management personnel who need the information for their duties.
- Laboratories, imaging providers, specialist clinicians, pharmacies, medication suppliers and other healthcare partners involved in your care.
- Couriers and delivery partners, who should receive only the minimum information needed for collection or delivery.
- Technology, website, form, cloud-storage, email, communication, cybersecurity and IT-support providers acting on our instructions.
- Payment processors, banks, accountants, auditors, insurers, professional advisers and debt-recovery providers where lawfully necessary.
- Government, regulatory, law-enforcement, public-health or judicial authorities where disclosure is required by law, court order, professional duty or a serious safety or public-health concern.
- A family member, caregiver, employer, insurer or other person only where you authorise the disclosure or another lawful basis applies.
We do not sell patient information. We do not provide health information to advertisers for their independent marketing.
9. Service providers and partner responsibilities
Some partners act as processors on our instructions, while independent healthcare providers may act as separate controllers for the clinical or professional services they provide. We seek written confidentiality, data-protection and security commitments appropriate to each relationship. A partner may also provide you with its own privacy notice where it independently determines how your information is processed.
10. International and cloud processing
Some technology and cloud-service providers may process or store information outside Nigeria. Where personal information is transferred internationally, InSitu Clinic will seek to use a lawful transfer basis and appropriate safeguards, such as recognised adequacy arrangements, contractual protections, access controls and security measures required by applicable law.
11. How we protect information
- Role-based access and confidentiality obligations for authorised staff and partners.
- Password protection, multi-factor authentication where available, device controls and controlled sharing permissions.
- Encryption or secure transmission where reasonably available and appropriate.
- Data minimisation, secure backups, incident management and periodic access review.
- Training, policies, contractual controls and disciplinary measures for inappropriate access or disclosure.
- Reasonable verification before responding to access, correction or disclosure requests.
No electronic or physical system can be guaranteed to be completely secure. Where a data breach creates a legally reportable risk, we will take appropriate containment, notification and remedial steps.
12. WhatsApp, email and telephone communications
You may select WhatsApp, email, telephone or another available method as your preferred communication channel. These channels may carry privacy risks, including access by anyone who controls your device, account, notification screen or shared email address. Please tell us if your contact details or privacy circumstances change. Do not use WhatsApp, email or website messages for emergencies.
We will use reasonable judgement about the sensitivity of information sent through each channel. Certain results, prescriptions or detailed clinical information may be provided through a more appropriate method or discussed during a consultation.
13. Retention and disposal
We retain information only for as long as reasonably necessary for the purposes described in this notice and in accordance with applicable clinical, professional, legal, tax, accounting, complaints, insurance and limitation requirements. The period depends on the type of information, whether you became a patient, the need for continuity of care, the possibility of a complaint or claim, and any legal hold or regulatory requirement.
Preliminary assessment information for a person who does not proceed may be retained for a limited follow-up and compliance period and then securely deleted or de-identified, unless a longer period is required for safety, legal or audit reasons. Marketing contact details are retained until you opt out or the purpose ends. When retention is no longer justified, records are securely deleted, destroyed or de-identified.
14. Your rights
Subject to applicable law and appropriate identity verification, you may have the right to:
- Be informed about how your information is processed.
- Request access to your personal information and, where applicable, a copy in a commonly used electronic format.
- Request correction of inaccurate, incomplete, outdated or misleading information.
- Request deletion or restriction where there is no overriding clinical, legal or other lawful reason to retain or continue processing the information.
- Withdraw consent where consent is the lawful basis, without affecting prior lawful processing.
- Object to processing based on legitimate interests and object at any time to direct marketing.
- Request data portability where the legal conditions apply.
- Request human review and contest a decision where a legally significant decision has been based solely on automated processing.
- Lodge a complaint with the Nigeria Data Protection Commission or seek another remedy available by law.
To make a request, email care@insituclinic.com with the subject "Privacy Request". We may ask for information to verify your identity and clarify the request. Some rights are not absolute, particularly where health records must be retained for care, safety, legal or professional reasons.
15. Children and persons lacking capacity
Our standard programme is for adults. If information concerning a child or person lacking capacity is processed, we will verify appropriate authority, involve the person as far as practicable, and apply the safeguards required for medical care, confidentiality and data protection.
16. Optional marketing, photographs and testimonials
Consent for marketing, photographs, videos, testimonials or promotional success stories must be separate from consent needed to assess or provide care. Refusing or withdrawing optional consent will not affect access to clinically appropriate services. No identifiable patient story, image or testimonial should be published without specific documented permission.
17. Complaints and contact
For privacy questions, requests or complaints, contact InSitu Clinic at care@insituclinic.com. We will investigate and respond appropriately. You may also lodge a complaint with the Nigeria Data Protection Commission.
18. Changes to this notice
We may update this Privacy Notice to reflect changes in our services, technology, partners or legal obligations. The current version will be published on our website with its effective date. We will provide additional notice where a change materially affects how existing patient information is used.
Terms of Service
By using our website, submitting an assessment, booking or paying for a service, or accepting care from InSitu Clinic, you agree to these Terms. Where a separate consent, treatment plan, quotation, invoice or partner condition applies, it forms part of the service arrangement to the extent that it does not conflict with applicable law.
1. About InSitu Clinic
InSitu Clinic provides doctor-led metabolic and weight-management services that may include online eligibility screening, physician review, telehealth consultations, home visits, clinical measurements, specimen collection, laboratory and imaging coordination, prescriptions, medication supply coordination, injection support, education and follow-up.
InSitu Clinic is operated by IN-SITU HEALTH ENTERPRISE.
2. Adults and service area
The standard InSitu Clinic programme is intended for adults aged 18 years and above. We may request proof of identity, age, location or authority to act for another person.
Services are available only in locations and jurisdictions where InSitu Clinic and its professionals are able to provide them lawfully and safely. You must accurately disclose your physical location during any telehealth consultation.
3. Preliminary assessment and eligibility
Submitting a form or speaking with our care team does not guarantee acceptance into the programme, a diagnosis, treatment, medication or prescription.
An InSitu Clinic clinician may decide that more information, physical examination, laboratory tests, imaging, specialist review, emergency assessment or care from another provider is required before any treatment decision.
We may decline, delay or discontinue a service where it is not clinically appropriate, is outside our scope, cannot be provided safely, or where required information, consent, payment or lawful documentation is not available.
4. Formation of the clinical relationship
General website information and automated acknowledgements are educational or administrative and are not a personal medical diagnosis or prescription.
A clinical relationship begins when InSitu Clinic accepts you for a clinical service and an authorised clinician assesses you. The scope of that relationship is limited to the services accepted and documented by the clinician.
5. Accurate and complete information
You must provide complete, accurate and current information about your identity, location, symptoms, diagnoses, medicines, allergies, previous adverse reactions, pregnancy or breastfeeding status, pregnancy plans, contraception where relevant, medical history and other information requested for safe care.
You must promptly inform us of changes, including new symptoms, hospital attendance, surgery, pregnancy, medication changes or treatment by another clinician. InSitu Clinic may rely on information you provide, and inaccurate or withheld information may delay care or cause harm.
6. Clinical judgement and treatment decisions
All prescribing and treatment decisions are made by an appropriately authorised clinician using independent professional judgement. Commercial preferences, stock availability or patient requests do not override clinical suitability.
A clinician may request further assessment, choose not to prescribe, recommend a different medicine, maintain a dose, delay dose escalation, reduce a dose, stop treatment or refer you elsewhere when this is considered necessary for safety or effectiveness.
7. Weight-management medicines
Prescription medicines, including semaglutide or tirzepatide where applicable, are supplied only following an appropriate clinical assessment and valid prescription.
Medicines must be used only by the named patient and exactly as directed. You must not share medicine, alter a dose, combine medicines, restart after an interruption or continue after being advised to stop without clinical guidance.
You must report significant side effects, severe or persistent symptoms, pregnancy and changes in other medicines. Product-specific counselling, contraindications, warnings and storage instructions form part of your treatment obligations.
Availability of a product, brand, strength or dose may change. If supply is interrupted, the clinician will decide whether to wait, use a clinically appropriate alternative or adjust the plan. There is no automatic entitlement to a specific medicine or brand.
8. Treatment results
No specific amount or rate of weight loss, improvement in blood results or other clinical outcome is guaranteed. Results vary according to individual biology, diagnosis, treatment response, dose, adherence, lifestyle, nutrition, activity, sleep, other medicines and medical conditions.
The clinical team may recommend discontinuation or a different approach if treatment is ineffective, unsafe, poorly tolerated or no longer appropriate.
9. Telehealth
Remote consultations may take place through video, telephone or another approved communication method. Telehealth has limitations and may not replace physical examination, diagnostic testing or emergency assessment.
You must read and accept the separate InSitu Clinic Telehealth Consent before receiving telehealth care. A clinician may stop a remote consultation and recommend an in-person assessment or emergency care.
10. Home visits and safety
You must provide an accurate address, safe access instructions and a reasonably private and safe environment for home services. You must disclose household hazards, security concerns, infectious symptoms, pets or other conditions that may affect the safety of staff.
InSitu Clinic may reschedule, modify or stop a home visit where the environment is unsafe, the patient cannot be identified, valid consent is absent, the required service is outside the professional scope, or another clinical or operational risk exists.
11. Investigations and partner services
Baseline and follow-up investigations are selected according to clinical history, examination, treatment and risk. The standard package may include HbA1c, lipid profile, kidney function, liver function, thyroid function and full blood count, with additional or fewer tests where clinically appropriate.
Laboratories, imaging providers, specialist clinicians, pharmacies, suppliers, couriers and other partners may provide part of the service. They may be independent regulated providers responsible for their own professional work, quality systems, privacy duties and results. InSitu Clinic remains responsible for the coordination and clinical decisions it undertakes.
12. Appointments and communication
You must provide working and privately controlled contact details and be available at the agreed time. You may choose an available preferred communication method, but certain information may need to be discussed through a more appropriate channel.
WhatsApp, email and telephone are not emergency channels. Messages are reviewed during stated business hours and response times may vary. Public social-media comments must not be used to send health information.
13. Fees, quotations and invoices
Current fees, inclusions, exclusions, taxes, delivery charges and payment timing will be stated on the applicable price schedule, quotation or invoice before payment.
The initial assessment, laboratory tests, medication, ongoing clinical management, injection support, delivery, specialist review and repeat investigations may be charged separately unless expressly included in a package.
Medication prices may vary by product, strength, quantity, supplier cost and availability. The exact prescribed medicine and dose can only be confirmed after clinical review. You will receive the applicable cost before treatment is supplied.
14. Payment
Payment is due by the date stated on the invoice. InSitu Clinic may require payment before a non-emergency home visit, laboratory booking, medicine order or other service is confirmed.
Failure or delay in payment may result in rescheduling or suspension of non-emergency services. This does not affect any statutory right to emergency care from an appropriate emergency provider.
15. Cancellations, rescheduling and refunds
Cancellation and rescheduling terms will be communicated at booking. Please provide reasonable notice if you cannot keep an appointment.
Where a service has not started, a refund may be available subject to any disclosed and reasonable non-recoverable cost already incurred, such as partner booking, transport, specimen materials, professional review or payment charges.
Services already performed, clinical reviews already completed, tests already ordered or collected, and professional time already used are generally not refundable, except where required by law or where InSitu Clinic agrees otherwise.
For patient safety, prescription medicines that have been dispensed, supplied or delivered cannot ordinarily be returned or reused. This does not remove your rights where the item supplied is incorrect, defective, damaged, counterfeit or otherwise fails to meet applicable consumer and medicines requirements.
No term in this section limits a right or remedy that cannot lawfully be excluded.
16. Patient responsibilities
- Follow clinical instructions, attend agreed reviews and complete requested monitoring.
- Store medicines safely, maintain cold chain where required, keep them away from children and other persons, and dispose of needles and sharps safely.
- Do not obtain duplicate or overlapping weight-management prescriptions without informing all treating clinicians.
- Treat staff and partners respectfully and provide a safe environment for care.
- Seek urgent or emergency assessment when instructed and do not wait for a routine InSitu Clinic response when symptoms are severe or rapidly worsening.
17. Emergencies
InSitu Clinic is not an emergency medical service and does not provide continuous monitoring. Severe chest pain, difficulty breathing, loss of consciousness, stroke symptoms, severe allergic reaction, severe hypoglycaemia, severe or persistent abdominal pain, uncontrolled vomiting, major bleeding, suicidal crisis or another emergency requires immediate assessment at an appropriate emergency facility or through local emergency services.
Do not rely on WhatsApp, email, website forms or a scheduled telehealth appointment for emergency care.
18. Privacy and records
Personal and health information is processed in accordance with the InSitu Clinic Privacy Notice and applicable health confidentiality and data-protection requirements.
InSitu Clinic creates and maintains clinical and administrative records. Access may be limited or redacted where necessary to protect another person, comply with law or preserve legally recognised professional information.
19. Optional marketing, photographs and testimonials
Marketing communications, photographs, videos, testimonials and promotional success stories require separate optional consent where applicable. Refusing optional consent will not affect clinically appropriate care. Consent may be withdrawn for future use, although withdrawal may not remove material already lawfully published or printed before the withdrawal is received.
20. Acceptable use and conduct
You must not misuse our website or communication channels, impersonate another person, submit false information, interfere with systems, threaten or abuse staff, seek medicines for diversion or resale, or use the service for unlawful purposes.
InSitu Clinic may suspend or terminate non-emergency services for fraud, abuse, threats, harassment, unsafe conduct, medication diversion, serious non-compliance or breakdown of the clinical relationship. Where clinically appropriate, reasonable steps will be taken to support continuity, handover or referral.
21. Website content and intellectual property
Website and educational content is general information and is not a substitute for individual assessment. Do not delay seeking care because of website content.
Unless otherwise stated, the InSitu Clinic name, branding, text, graphics, forms and materials belong to the legal operator or are used with permission. They may not be copied, republished, sold or commercially exploited without written permission.
22. Third-party links and systems
Our website or communications may link to third-party websites, payment systems, forms or partner services. We are not responsible for independent third-party content, availability or privacy practices, although we take reasonable care in selecting partners used to deliver our services.
23. Service availability and events beyond control
Service availability may be affected by clinician availability, weather, traffic, power or internet failure, laboratory delays, product shortages, courier disruption, regulatory action, public-health events, security incidents or other circumstances beyond reasonable control. We will take reasonable steps to communicate and provide an appropriate alternative, reschedule or refund for an undelivered service where applicable.
24. Responsibility and limitation of liability
InSitu Clinic will use reasonable professional skill, care and judgement in the services it provides. Medical care is not risk-free, and outcomes cannot be guaranteed.
To the extent permitted by law, InSitu Clinic is not responsible for harm caused by inaccurate or withheld information, unauthorised medicine sharing or dose changes, failure to follow safety instructions, use of unapproved or independently sourced products, or delay in seeking emergency care.
Nothing in these Terms excludes or limits liability for fraud, wilful misconduct, negligence causing death or personal injury, breach of professional duty, or any consumer, patient or statutory right that cannot lawfully be excluded.
25. Complaints
Questions or complaints may be submitted to care@insituclinic.com. Please provide your name, patient number if available, the relevant dates and a clear description. We will acknowledge, investigate and respond in line with our complaints process.
You may also complain to an appropriate professional regulator, consumer-protection authority, data-protection authority or court where applicable.
26. Governing law and disputes
These Terms are governed by the laws of the Federal Republic of Nigeria.
The parties should first attempt to resolve a dispute through the InSitu Clinic complaints process and good-faith discussion. This does not prevent either party from seeking urgent relief, reporting a regulatory concern or using a court or other remedy available by law.
27. Changes to these Terms
We may update these Terms to reflect changes in law, clinical practice, partners, pricing structure or service operations. The current version will be published with its effective date.
The version applicable to a paid service will generally be the version made available when the service was booked, unless a change is required by law, regulator direction or urgent safety considerations.

How you accept these Terms
You do not sign anything on this page. When you complete the eligibility assessment or book a service, you will be asked to confirm the following statement before proceeding:
Telehealth Consent
Telehealth means providing healthcare when the patient and clinician are in different locations using video, telephone or another communication technology. Telehealth may be used for preliminary review, physician consultation, follow-up, treatment monitoring, medication counselling, review of results and other clinically appropriate services.
1. Nature and scope of telehealth care
An InSitu Clinic clinician may assess symptoms, review your history, medicines, measurements, photographs or documents you choose to provide, discuss investigation results, provide education, make clinical recommendations and, where lawful and clinically appropriate, issue or renew a prescription.
Telehealth is only one part of the InSitu Clinic service. Home assessment, laboratory testing, physical examination, imaging, specialist referral or hospital care may also be required.
2. Potential benefits
- Convenient access to a clinician without travelling to a clinic.
- Timely review of results, treatment response, side effects and dose decisions.
- Improved continuity and coordination between the physician, concierge nurse and patient.
- Reduced travel time and more flexible follow-up.
3. Limitations and risks
- The clinician may be unable to perform a complete physical examination or directly verify all signs and measurements.
- Video, sound, photographs, documents or patient-reported information may be incomplete, inaccurate or poor quality.
- A condition may be missed, misunderstood or require an in-person examination, investigation or specialist assessment.
- Technology may fail, disconnect or delay communication.
- Electronic communication carries a risk of unauthorised access, interception, device loss, shared account access or accidental disclosure.
- A remote consultation may not be suitable for an emergency, rapidly worsening illness or a condition requiring hands-on assessment.
4. Identity, location and emergency details
At the start of a consultation, you may be asked to confirm your full name, date of birth, telephone number, current physical location and an emergency contact.
You must tell the clinician if you are outside Nigeria or outside the service area. The clinician may be unable to provide care or prescribe across a jurisdiction where the professional is not authorised to practise.
If an emergency develops, the clinician may advise you to contact local emergency services, attend the nearest appropriate hospital, involve a person with you or permit contact with your emergency contact or another provider where lawful and necessary for safety.
5. Privacy and confidentiality
The clinician will conduct the consultation from a setting intended to protect confidentiality. You should participate from a private location where you can speak freely and where your device and account are not accessible to an unauthorised person.
Personal and health information will be processed in accordance with the InSitu Clinic Privacy Notice, applicable data-protection law and professional confidentiality duties.
Other authorised members of the care team may access the clinical record or participate where necessary for care, documentation, interpretation, supervision or technical support. You will be informed where another person is actively present during the consultation unless an urgent safety situation makes prior notice impracticable.
6. WhatsApp, email and messaging
WhatsApp, email and telephone may be used for appointment coordination and, where appropriate, limited clinical communication. These channels may display notifications or content to anyone with access to your device or account.
Do not send emergency messages through WhatsApp, email, social media or website forms. Do not send another person's health information unless you are authorised to do so.
The clinician may decide that a matter is too sensitive or complex for text messaging and require a telephone, video or in-person consultation.
7. Recording and screenshots
InSitu Clinic will not intentionally audio-record or video-record a consultation unless you are specifically informed and give separate consent, except where recording is required by law and lawfully authorised.
You must not record, livestream, photograph or take screenshots of the clinician or consultation without the clinician's prior agreement. This protects the privacy of all participants and any third-party information that may appear.
8. Clinical information and patient responsibilities
- Provide complete and accurate information about symptoms, diagnoses, medicines, allergies, pregnancy or breastfeeding status, pregnancy plans, recent procedures and other care.
- Use a reliable device and connection where possible, check audio and video before the appointment, and keep the device charged.
- Ensure that measurements or home devices used during the consultation are appropriate and report who obtained the measurement and how.
- Ask questions when information or instructions are unclear.
- Follow instructions for laboratory tests, medicines, follow-up and emergency assessment.
- Tell the clinician promptly if symptoms worsen, a medicine causes a significant adverse effect or your circumstances change.
9. Prescribing and medicines
Consent to telehealth does not guarantee that a medicine will be prescribed or renewed. The clinician may require a home or in-person assessment, laboratory tests, identification, records from another provider or specialist review before prescribing.
Where a prescription is issued, you must follow the dose, storage, monitoring and safety instructions. Dose changes should occur only after a documented clinical decision.
10. Physical examination and home assessment
You may be asked to show a visible area on camera or perform a simple self-examination or movement. You may decline. The clinician will explain the purpose and limitations and may recommend an in-person or home assessment instead.
A concierge nurse or another authorised professional may be arranged to obtain blood pressure, pulse, weight, BMI, waist circumference, specimens or other measurements. Separate consent will be obtained for a home visit, examination or specimen collection as appropriate.
11. Technical failure
If the video connection fails, the clinician may attempt to reconnect, telephone you, reschedule or direct you to another form of care. You should keep your telephone available and ensure the contact number provided is correct.
A technical interruption may reduce the quality of assessment. The clinician will decide whether enough information was obtained to provide advice safely.
12. Emergencies and urgent symptoms
Telehealth is not a substitute for emergency services. Severe chest pain, difficulty breathing, loss of consciousness, stroke symptoms, severe allergic reaction, severe hypoglycaemia, severe or persistent abdominal pain, uncontrolled vomiting, suicidal crisis or another medical emergency requires immediate care at an appropriate emergency facility or through local emergency services.
Do not wait for a reply from InSitu Clinic if symptoms are severe or rapidly worsening.
13. Choice, alternatives and withdrawal
You may refuse telehealth and ask whether an alternative is available. Alternatives may include a home visit, in-person consultation with another provider, referral or rescheduling. Availability, location and fees may differ.
You may ask to stop a telehealth consultation or withdraw consent to future telehealth care at any time. Withdrawal will not affect care already provided, lawful processing already undertaken, fees for services already performed or clinical records that must be retained.
The clinician may stop the consultation if telehealth is not safe, appropriate or technically adequate.
14. Chaperone, support person and interpreter
You may request a chaperone, interpreter or support person, subject to availability and confidentiality arrangements. Please tell us before the appointment where possible.
If another person is present with you, you should identify them and confirm that you agree to their presence and any information shared in their hearing.
15. Questions and complaints
You may ask questions before or during the consultation. Concerns or complaints may be sent to care@insituclinic.com or through the official contact details above. Privacy complaints may also be directed to the Nigeria Data Protection Commission, and professional concerns may be directed to the relevant healthcare regulator.
How this consent is given
You do not sign anything on this page. Before your first telehealth consultation, our care team will send you this consent to confirm electronically (or on paper during a home visit). You will be asked to confirm each of the following statements:
